Transition Assessments

An evaluative tool to determine strengths, preferences, interests and needs.

The Transition Assessment will help the individual and their family members make informed decisions about areas of focus for training and skill building for their life.

The assessment process gathers meaningful information in four core areas:

  • Independent living skills
  • Community involvement
  • Self-determination
  • Employment exploration

The results from the assessment may be used to develop meaningful transition goals and objectives for an Individualized Education Plan (IEP) or for service providers to identify focus areas for training and skill acquisition in adulthood.
