Program Counselor
Program Counselors at Vista Life Innovations (Vista) are dedicated professionals who provide individualized support and guidance to help students and members thrive in all areas of life. Acting as case managers and direct service providers, Program Counselors develop and implement personalized plans that promote independence, emotional well-being, and community engagement.
Core Responsibilities:

Create and maintain written service plans tailored to individual goals and progress, including the Individual Program Plan (IPP) and Individual Progress Review (IPR)

Support members in four key areas: social/emotional development, vocational readiness, life skills, and community involvement

Deliver direct instruction in areas such as time and money management, medication routines, and social skills

Provide case management services to individuals and families, fostering emotional health and stability

Monitor medical, wellness, and financial needs while ensuring proper documentation and communication

Collaborate with staff, families, and external providers to ensure a holistic approach to care

Serve as a positive role model, advocate, and resource for students and members navigating daily life and challenges

Engage proactively in crisis management and problem-solving when needed
